Abstract

53歳,男性.フィリピンからの帰国時より左小指の腫脹,発疹,両上肢遠位部の関節痛と強ばり,右肩痛,発熱がみられた.身体所見では右肩関節の可動制限と両小指に直径3mm程度の中心紫色で周辺やや発赤した丘疹を各一個認めた.核左方移動を伴った白血球増加とCRPの軽度上昇を認めたが他に異常なし.入院後右手関節屈側の腱鞘炎も出現.血液培養から淋菌を検出し播種性淋菌感染症による敗血症と診断.セフェム系抗菌薬投与にて治癒.性感染症予防対応を患者に伝えた.
