Abstract

片麻痺，意識障害，痙攣を来たした17歳，男性．頭部MRI T2WIで両側前頭葉皮質，大脳基底核に多発する高信号域，脳血管造影で両側ACA近位部に分節性の血管狭窄がみられた．RCVSとの鑑別に苦慮したが，髄液異常があり，発症3カ月後でも血管狭窄が残存したことからPACNSと診断した．両側性，複数の血管支配域におよぶ若年性脳梗塞ではPACNSを鑑別に挙げ，髄液検査と脳血管造影を施行する必要がある．
