We encountered two cases of a subtype of malignant hyperthermia in which marked myoglobinuria and sharply increased serum CPK values in excess of 100, 000 IU occurred. To treat the myolysis-induced myoglobinuria and to prevent metabolic products causing acute renal failure, we instituted hemofiltration 24 hours after the onset of mioglobinuria in one case and 8 hours post-onset in the other case, and obtained favorable results. Acute renal failure remains a significant complication in malignant hyperthermia and it is a major factor influencing the outcome of patients. Early hemocatharsis should be instituted on the basis of the patient's status, their serum CPK levels, and their clinical course.
