<p>【Objective】The aim of the present study was to elucidate the effect of collaboration between the medicine consultation room and a polypharmacy clinic on fall prevention.【Methods】We reviewed the effect of fall prevention from January 2017 to March 2017 in our hospital. We recommended patients using the medicine consultation room to visit a polypharmacy clinic. The patients had a history of falls more than once within one year and took more than 10 types of internal medicines per day. Apharmacist picked the medicine that can cause falls and examined the fall risk. We provided detailed information on patients who agreed to the proposal of prescription disposition to prescribing physicians and family doctors. Three months after using the medicine consultation room, we asked questions about the degree of satisfaction of using the medicine consultation room and conducted a questionnaire survey on the existence of falls.【Results】There were four subject cases in this study. All cases were over 80 years old and were on antihypertensive medication. Drowsiness, wobbling, low attention, fainting, and dizziness tended to be showed more frequently among the commonly reported side effects which can cause falls. The medicine consultation room was highly evaluated for the purpose of this study. After using the medicine consultation room all subjects did not fall for 3 months, and symptoms such as stumbling and wobbling reduced.【Conclusion】We have not performed statistical analysis in this study because the number of cases was small and the follow-up period of the research was short. It was suggested that collaboration between the medicine consultation room and polypharmacy clinic in our hospital could prevent falls, because the falls ceased andsymptoms such as wobbling and stumbling also reduced.</p>
