Abstract

A 57-year-old woman presented with a 1-year history of reticulated hyperpigmentation on the left cheek. Three months later, she visited our hospital because it similarly appeared on the right cheek and gradually increased. Physical examination demonstrated asymptomatic brown pigmented patches on both cheeks. Skin biopsy revealed epidermal atrophy and liquefaction degeneration, and numerous eosinophilic necrotic cells. Additionally, many lymphocytes and melanophages were observed in the superficial dermis. She had been treated with diltiazem hydrochloride for 8 months prior to presentation. We diagnosed her with diltiazem-associated photodistributed hyperpigmentation based on the characteristic clinical and histological features. After discontinuation of diltiazem hydrochloride, sun screen and topical tacrolimus were applied, and the reticulated hyperpigmentation on her face gradually faded. DAPH is a rare disease. If the characteristic reticulated, slate-gray hyperpigmentation on sun-exposed areas is observed and the patient has taken diltiazem hydrochloride for a long time, this disease should be considered during treatment.Skin Research, 16: 411-416, 2017
