<p>Diabetic neuropathy is a common complication of diabetes in the early phase of the disease. Severe diabetic neuropathy is associated with the development of diabetic foot. Numerous metabolic abnormalities caused by hyperglycemia lead to the onset of diabetic neuropathy. Furthermore, recent studies suggest that type I and type II diabetes may also cause the onset of the disease through different mechanisms. Patients with diabetic neuropathy present with positive symptoms, such as numbness and pain, in both feet. In addition, many patients experience symptoms that go undetected by themselves and their physicians. These symptoms include reduced sensitivity (negative symptom) and worsening of autonomic dysfunction, which lead to poor outcomes. Therefore, early diagnosis and appropriate treatment strategies that address the root cause of the symptoms are necessary. In Japan, simplified diagnostic criteria for diabetic neuropathy were proposed, including medical interview, Achilles tendon reflex, and vibration perception threshold test. However, these criteria have not been used widely. Internationally, assessment of thermal nociception and monofilament testing are recommended, in addition to the diagnostic tests recommended in the simplified criteria. It is important to ensure that at least the procedures recommended in the simplified diagnostic criteria are performed for all patients with diabetes.</p>
