Gallbladder Metastases of Invasive Ductal Carcinoma of the Breast Resected 9 Years after Initial Mastectomy

Abstract
<p>A 56-year-old woman who underwent total mastectomy with axillary lymph node dissection for right breast invasive ductal carcinoma 9 years previously, was found to have liver and bone metastasis within 3 years after the operation. These lesions were controlled and remained small by multimodal therapies. Six years after the operation, a small mass appeared in the gallbladder. Nine years after the operation, elevation of CEA (45.6 ng/ml) was detected and PET scan showed intense uptake in the gallbladder. Imaging tests revealed a sessile polypoid lesion of 2.5 cm in its body. To avoid tumor implantation, we did not perform a needle biopsy but performed cholecystectomy and gallbladder bed resection. Pathological examination revealed alveolar or trabecular proliferation of adenocarcinoma throughout the gallbladder wall as well as liver invasion and lymph node metastases. Its endocrine receptors and HER2 expression patterns were consistent with those of the initial breast cancer. In the literature, there are 28 cases of gallbladder metastases from breast cancer, of which only 6 were invasive ductal carcinoma. Even years after the operation, metastases from breast cancer should be included in the differential diagnosis of a newly appeared lesion in the gallbladder.</p>
