Abstract

Background. Bronchial foreign bodies are commonly reported, but reports of concrete fragments in bilateral bronchi are rare. Case. A 55-year-old man lost consciousness during demolition work, and became aware of difficulty in breathing after he recovered. A computed tomography showed foreign bodies in the right middle and the left main bronchi. We prepared for progression of respiratory failure by extracorporeal membrane oxygenation (ECMO) before treatment. A flexible bronchofiberscopy showed concrete fragments in bilateral bronchi, which we unsuccessfully attempted to remove with alligator forceps and basket forceps. We then successfully removed the concrete fragments from both bronchi using a tripod grasping forceps. Conclusion. There are few case reports of foreign bodies in the bilateral bronchi and to the best of our knowledge, there are no previous adult cases. The foreign objects in bilateral bronchi were successfully extracted with a flexible bronchoscope after carefully considering extraction methods and taking measures against respiratory failure.
