Encephalocele through Sternberg’s Canal: A Case Report

Abstract

<p>Intrasphenoidal encephalocele with idiopathic cerebrospinal fluid (CSF) leakage is an extremely rare finding. Sternberg’s canal is a remnant of the craniopharyngeal canal that is formed due to incomplete fusion of the sphenoid bone. After development of the sphenoid sinus in adults, a fistula forms between the middle coronal plane and sphenoid sinus, and meningoencephalocele through Sternberg’s canal may arise.Here, we present a case of encephalocele through Sternberg’s canal. The patient was a 53-year-old man who suffered from CSF rhinorrhea and recurrent fever and headache. He was referred to our department for these symptoms after conservative treatment at an ENT clinic. Encephalocele through Sternberg’s canal was suspected on computed tomography (CT) and magnetic resonance imaging (MRI). Encephalocele was diagnosed based on a elevated levels of glucose in the rhinorrhea, and resection of the encephalocele and reconstruction of the skull base were performed endoscopically. In this case, there were some complicated anatomical variations, such that the maxillary nerve was running at the bottom of the lateral fossa of the sphenoid sinus and the posterior wall of the maxillary sinus was thickened. Therefore, we needed contemplation of the surgical approach before the operation. There has been no recurrence of CSF leakage in 2 years and 3 months after surgery.</p>
