Description
A contact study of the radiohumeral joint using 62 osteoligamentous elbows demonstrated that (1) the largest contact area was often observed in pronation, and the smallest in supination, but the majority of specimens did not show a constant increase in contact area during supination-pronation and showed various paradoxical patterns; (2) smaller contact areas located more peripherally in pronation were observed after longer migration of the geometrical center; (3) the geometrical center in pronation tended to disperse in specimens with restricted degeneration, but converged into the central area in those with extended degeneration. These results strongly suggest paradoxical and unstable rotation of the radial head during supination-pronation, not only in degenerative specimens but in approximately a half of the non-degenerative population. We considered that the latter group, i.e. those with masked abnormalities, were undergoing a process of compensation to maintain a sufficient range of flexion-extension movement at the elbow.(Received September 11, 2001 and accepted October 2, 2001),肘の回内・回外運動時における腕橈関節接触面の位置と面積を,62肘の解剖体標本を用いて検討した結果,次のような所見を得た.1)回内時に接触面が広く,回外時に狭い.しかし,回内・回外運動中の面積の変化は一方向的ではなく,奇異な変化を含めて様々な個体差を示した; 2)回内時において接触面が狭い例では,関節面の辺縁部で接触するばかりでなく,回外に向けて接触面(同面の幾何学的中心)が移動する距離が大きかった; 3)回内時における接触面の幾何学的中心の位置は,関節軟骨の変性が限局している群において特に個体差が大きかった.しかし軟骨変性が広範囲に及ぶと,接触面は関節の中心に集まった.以上の結果は,軟骨変性個体ばかりでなく,奇異な接触面積の変化を示す正常群の約半数においても,回内・回外運動時において,橈骨頭が不規則で不安定な運動を行うことを示唆している.正常群における橈骨頭運動の異常は,靭帯等の加齢変化が進行する中で,肘の屈曲・伸展運動を維持するための代償作用であると考えた.
