目的: 18トリソミー患者に対する積極的治療介入が,在宅移行や生命予後に影響するか検討した.対象および方法: 2000年1月から2018年12月に当院で経験した18トリソミー37例を対象とした.外科的治療介入も選択肢となった2010年を区切りに,2000年1月から2009年12月までの前期群17例,2010年1月から2018年12月までの後期群20例を,在宅移行や生命予後について診療録より後方視的に比較検討した.結果: 患者背景に有意差は認めなかった.積極的治療介入例は内科的,外科的治療共に後期群で有意に増加していた.生存退院や1か月以上の生存例も後期群で有意に増加した.1年以上の生存例では有意差は認めなかった.後期群では家族が積極的治療介入を希望した症例14例中11例が生存退院していた.結論: 18トリソミー患者に対する積極的治療介入は,児の生存期間の延長,及び生存退院率の向上につながっている.Objective: We aimed to assess the change in discharge rate and prognosis of patients with trisomy 18 after the introduction of positive medical and surgical interventions. Materials and Methods: The clinical records of 37 patients with trisomy 18 who were admitted to our hospital between January 2000 and December 2018 were retrospectively analyzed. The patients were divided into two groups according to the following treatment period: from January 2000 to December 2009 (n = 17) and from January 2010 to December 2018, when surgical treatment was introduced (n = 20). Results: No significant differences in the background characteristics were noted between the two groups. For cases requiring positive medical and surgical interventions, the frequency of both medical and surgical interventions increased significantly in the 2nd period. The survival at the time of hospital discharge and the survival rate at >1 month increased significantly in the 2nd period. No significant difference in the survival rate at >1 year old was noted between the two groups. In the 2nd period, 11 out of 14 patients whose families hoped for positive medical and surgical interventions could be discharged. Conclusion: Positive medical and surgical interventions improve the survival and live-discharge rate in patients with trisomy 18.
