重度の運動麻痺と高次脳機能障害を有する脳腫瘍患者の入院から自宅退院に至る過程を,主介護者へのインタビューと共に振り返り,自宅復帰の支援のあり方を検討した。 インタビュー結果から,感情のコントロール,社会資源の利用,家族の協力,介護以外の役割,葛藤などの問題点が抽出された。 脳腫瘍患者の自宅復帰に向けた介護者への支援のあり方としては,1介護者の精神的健康,心理面への配慮,2その場に応じた適切な症状の説明と対処方法の指導,3介護者の社会的な役割の把握,4介護者,医師,看護師,ソーシャルワーカー,リハビリテーション従事者のチームでの関わりの4点が重要であることが示唆された。

We investigated how to support caregivers who try patients with brain tumor, severe motor paralysis and higher brain function disturbances to discharge by interviewing the process from admitting to discharging a hospital. The results indicated the problem of emotional control, use of social resources, family cooperation, other roles caregivers, conflicts of care givers etc. and the following four points were very important for caregivers.1) Mental health or psychological care for caregivers, 2) Inform appropriate explanation of patient's symptoms and teach caregivers to how to cope with this, 3) Understanding of social role of caregivers, 4) Team approach to caregivers, composed by medical doctors, nurses, caregivers, social workers, rehabilitation professionals etc. is critical.
