Abstract

<p>甲状腺手術における頸部郭清術の中で内視鏡手術の占める割合はまだまだ少なく，これから発展していく分野である。内視鏡下の頸部郭清では術者が鉗子操作に十分習熟していることが必要不可欠であるが，内視鏡の鮮明な拡大視効果で神経や局所解剖も良好に視認できるため開創手術とほぼ同等の郭清が可能である。取り残しのある不十分な郭清を避けるためには，気管や総頸動脈・内頸静脈を効果的に牽引して有効なWorking spaceと良好な視野を確保する必要がある。この効果的な牽引こそが最も重要である。また，１方向からの操作だけでなく異なる方向からの鉗子操作を併用することで郭清領域が格段に広がり，正確な外側区域郭清も可能となる。</p>
