<p>The oncoplastic breast-conserving surgery (OPBCS) working group has proposed a step-up guide for learning system for OPBCS. Following the Basic OPBCS, Volume displacement Step 1 will be explained.</p><p>In addition to dual-plane undermining for glandular rearrangement to fill the partial mastectomy defects, (1) hiding incisions in aesthetically pleasing positions, and (2) re-centralization of the nipple-areolar complex (NAC re-centralization) may be possible to further improve the cosmetic outcome and prevent the deviation and deformity of the NAC. Then we set these techniques as Volume displacement Step 1.</p><p>One of the typical late complications after partial mastectomy is deviation and/or deformity of the NAC. To prevent this complication, de-epithelialization around the areola is used in combination to move the NAC in the opposite direction to partial resection. De-epithelialization and NAC re-centralization are basic and important techniques that can open the door to performance of more advanced OPBCS.</p>
