A Case of Skin Metastasis of Digital Papillary Adenocarcinoma with Clinical Appearance of Hard Edema

<p>56 歳，男性。左母指紅色結節を切除し，digital papillary adenocarcinoma と診断され，経過観察中であった。手術から 35 カ月後，左腋窩リンパ節転移を認め，左腋窩リンパ節郭清術を施行した。郭清術施行後，左上肢全体のリンパ浮腫をきたした。その後，徐々に左上肢は硬性浮腫となり，腋窩郭清から 28 カ月後，上肢の一部に紫斑，色素沈着が出現した。また，左小指外側に骨様硬の皮下硬結を認めた。前腕部より皮膚生検を行ったところ，異型を伴う腫瘍細胞が管腔構造を形成していた。PET-CT では，左手掌・左小指外側，左手掌～上腕遠位の皮下，左腋窩部に集積を認めた。その他の部位に集積は認めなかった。以上から digital papillary adenocarcinoma の多発皮膚転移と診断した。Digital papillary adenocarcinoma は緩徐進行性の汗腺系悪性腫瘍であり，自験例は初診時の臨床像や病理組織像は典型的であったが，後に特殊な転移様式を呈した。</p>
