Abstract
<p>A 27-year-old woman with atopic dermatitis had a history of exercise-associated urticaria starting when she was 20. Once she experienced urticaria, nausea, and vomiting after playing basketball for 30 min. She subsequently developed dyspnea and syncope due to hypotension. Her anaphylaxis was not associated with food intake. Because she had a strong desire to exercise safely, we tested her with a thermal stimulus under resting conditions and exercise challenge with serum histamine and tryptase. The results ruled out cholinergic urticaria. Instead, she was diagnosed with exercise-induced anaphylaxis triggered by an exercise load more than of 80 kJ. A series of provocation tests is useful for diagnosis and determining the tolerable limit of exercise load for preventing anaphylaxis in patients with exercise-induced anaphylaxis.</p>
