Abstract

An 89-year-old man found collapsed at home was transferred to our hospital. Initially, his consciousness level was JCS III-300 and vital signs, stable. Subcutaneous hematoma around the left eye and a 3-mm contusion in the left cheek were observed. Computed tomography (CT) revealed fronto-orbital fracture, brain hemorrhage with ventricular perforation, pneumocephalus, and brain contusion. He was treated conservatively, but died on hospital day 2. Imaging examinations, including CT, should be performed in all cases of head and face impalement injuries, irrespective of severity, to identify the entry route of foreign objects and diagnose organ injury.
