The purpose of the present study was to investigate function and size of the quadriceps femoris (QF) of a patient who gave an operation on one's knee joint. A forty-seven years old female volunteer engaged in this study, who had disc remove operation from her right knee joint seven months ago. Maximal voluntary contraction (MVC) during knee extension under isometric condition was measured. Using muscle functional magnetic resonance imaging (mfMRI), axial images of the operated and non operated limbs was taken at rest and immediately after repetitive isotonic knee extension exercises (RIKEE), consisted of 5 sets of IO reps with 1 min rest between each sets, with a load equal to 25% of MVC. Spin-spin relaxation time (T2), which represented mfMRI signals, of the QF was calculated as an index of muscle activation during the RIKEE. Resting image of the thigh was also used for calculation of cross-sectional area (CSA) of the QF and hamstrings. During the RIKEE, surface electromyography (EMG) activity was measured from the rectus femoris (RF), vastus lateralis (VL) and vastus medialis (VM) with a sampling frequency of 1 kHz with band pass filter of 15 Hz to 500 Hz using pre-amplified electrodes. MVC and CSA of the QF in the operated limb were corresponded to 46% and 81%, respectively, of the non-oparated limb. mfMRl signal change after the RIKEE in the operated limb (22%) was greater than non-operated limb (8%). These results suggest that severe muscle deconditioning, i.e. depression of neuromuscular system and atrophy, is still induced by knee surgery after 7 months of operation in this case.
