<p>We report a case of prolonged loss of consciousness due to tizanidine hydrochloride overdose. A man in his 50s with depression and back pain was brought to the hospital after tizanidine hydrochloride overdose (52 mg). His presenting consciousness level was GCS : E1V1M1, with bradycardia and pupillary constriction. After 10 hours, his consciousness level improved. Although the drug’s half-life is 1.5 h, the prolonged unconsciousness was caused by the large dose and the drug’s slow metabolism. Moreover, the parasympathetic nervous system became dominant, resulting in cholinergic toxidrome-like symptoms. In conclusion, tizanidine hydrochloride overdose should be considered in patients exhibiting these features.</p>
