- CT西中分類と術中のempty signにより骨軟骨片の温存が可能と判断した上腕骨小頭離断性骨軟骨炎の1例
Abstract
【緒言】上腕骨小頭離断性骨軟骨炎(以下OCD)に対する治療は術前画像評価と術中のICRS分類によって決定するが,ICRS分類では骨軟骨片の組織強度は考慮にない.今回,CT西中分類と術中empty signを基に骨軟骨片の温存が可能と判断した1例について報告する.<BR> 【症例】15歳男児,左肘痛.半年前から左肘痛を認め,単純X線岩瀬分類で遊離期巣内型,CT西中分類でstage3aのため手術の方針とした.術中所見ではICRS分類OCDⅢだったが,empty signは陰性であった.骨軟骨片の温存が可能と判断し,吸収ピンで骨軟骨片を固定した.3週間の外固定を行い,術後3カ月から投球動作を開始した.部活内のトラブルで野球からは離れたが,術後1年9カ月で良好な骨癒合が得られた.<BR> 【考察】CT西中分類で軟骨下骨の連続性が残存し,術中のempty signが陰性であれば,組織の温存が可能と考えられる.
