Description

<p>The concept and chronology of "healthcare transition" for patients with childhood-onset chronic diseases were proposed for the first time by the Japan Pediatric Society. In this report "Transition support" has expanded beyond medical care and includes a broader perspective on patient health and welfare to help patients become independent and functioning adults of society. The Japan Pediatric Society proposed 20 items related to four main items (basic attitude, comprehensive support with a lifelong perspective, support for changing departments, system development, and other necessary measures) to promote transition support and emphasize that pediatric physicians be familiar with the importance of transition support.</p>
