Nationwide survey of analysis of poor prognostic factors for living donor liver transplantation for hepatocellular carcinoma.

<p>Background: Current insurance coverage for liver transplantation (LT) for hepatocellular carcinoma (HCC) is based on the Milan criteria or the 5-5-500 criteria. We have analyzed risk factors based on updated nationwide data from Japan.</p><p>METHODS: Subjects were 516 LDLT cases for HCC performed between 2010 and 2018 at 37 centers nationwide. Cox proportional hazards model was used to analyze prognostic factors on recurrence-free survival rate in 474 cases within the new Japan criteria.</p><p>Results: The 5Y-OS and RFS rates for patients within and outside the new Japan criteria were 81% vs. 56% and 77% vs. 44%, respectively. The RFS for patients within the Milan criteria and outside the 5-5-500 criteria were worse compared with within the new Japan criteria. Multivariate analysis identified NLR 5 or higher and prior hepatectomy as independent risk factors.</p><p>Conclusions: The results of the nationwide survey showed that the new Japan criteria were valid, but as in previous studies, patients within the Milan criteria and outside the 5-5-500 criteria had a poor outcome. Poor prognostic factors for patients within the new Japan criteria were NLR 5 or higher and prior hepatectomy.</p>
