<p>Tamoxifen is firstly recommended for hormone positive advanced male breast cancers. On the other hand, CDK 4/6 inhibitors are mainly used together with endocrine therapy for hormone positive advanced breast cancers. We report a case of advanced male breast cancer treated with aromatase inhibitor combined with CDK 4/6 inhibitor. A 72-year-old man presented with a right thoracic mass with self-destruct and bleeding. Respiratory distress and anemia (Hb ; 5.0 g/dl) were noted, and blood tests showed elevated tumor markers. The pathological examination showed invasive ductal carcinoma (tubule forming) with positive for ER and PgR, negative for HER2, and 40 % for MIB-1 labeling index. There were multi-organ metastasis (lung, bone, and lymph nodes) on PET-CT scan and duodenal benign ulcer with upper gastrointestinal endoscopy. Chemotherapy was avoided because of his risk for ulcer perforation, serious anemia and decreased pulmonary function. Therefore, an endocrine regimen with Letrozole plus Palbociclib was started and produced a good response. This treatment is expected to be effective for male as well as female breast cancers in hormone-positive patients.</p>
