- ベアメタルステント留置半年後に冠動脈瘤様の造影所見を認めた1例

Description
症例は60歳代, 男性. 10年前より高血圧, 脂質異常症にて内服加療を受けていた. 突然の胸痛が出現し, 当院へ搬送され, 急性心筋梗塞の診断にて心臓カテーテル検査を施行された. 冠動脈造影にて#7 90%狭窄を認め, ベアメタルステント (bare metal stent ; BMS) を留置され, 後拡張を行い血管内超音波 (intravascular ultrasound ; IVUS) にて良好なステントの圧着を確認し, 手技終了した. その後順調に経過していたが, 6カ月後に確認造影を行ったところ, ステント留置部に動脈瘤様の所見を認めた. IVUSを施行したところ, 急性期と比し血管径とステント径に大きな変化を認めなかったが, ステントと血管壁の間に血流所見を認め, incomplete stent apposition (ISA) と診断した. 同部に対してバルーンにて可及的にステントを十分に拡張した. BMS留置後にlate acquired ISAを認めた興味深い症例と考えられた.
