- 巨大心室瘤心外膜面を右室側から左室側にかけて横断する緩徐伝導を証明し得た心室頻拍の1例

Abstract
<p> 症例は非虚血性心筋症の61歳女性. 2006年にVTを発症しICDを植込み, 過去2回心内膜側よりカテーテルアブレーションを実施するもVT残存あり, 今回は心外膜側よりアプローチした. 左室基部に巨大心室瘤を, 両心室心外膜側には広範にlow voltage area (LVA) を認めた. 右室期外刺激にてVT1 (VTCL 330ms) が誘発された. VT1中に左室中部前壁にmid diastolic potential (MDP) を認め, concealed entrainment (CE) が得られVT回路上と判断, 通電にてVT1は停止した. さらにVT2 (VTCL 430ms) が誘発され, LV基部のLVAで広範にMDPを認めCEが得られたが, 幅広く通電するもVT2は停止しなかった. 一方, 右室側ではLVZ内にearly diastolic potentialを認め, CEも得られ右室側がentranceと考えられた. 右室側の通電にてVT2は停止し, 同部位で洞調律中のdelayed potentialを確認, 以後VTは誘発不能となった. VT2は心室瘤右室側に比較的幅の狭いentrance, 左室側に幅広いExitを有するVTと考えられた.</p>
