Endoscopically defined complete healing (white scar;WS) rates of gastric ulcers (GU) with cimetidine, and its dosage to introduce WS from its incomplete (red scar;RS) healing were investigated. One hundred thirty-nine patients with active stage gastric ulcer were randomized by an envelope method into two study groups, A and B ; patients in group A received 800mg/day (BID) of cimetidine until their ulcers reached WS, while those in B received 800mg/day up to RS and after then 400mg/day at bed time until the scars reached WS. Of 139, 110 eligible patients for analysis, 56 in group A and 54 in B, were well randomized, where no statistical difference was noted in the patients' backgrounds. Serial cumulative WS healing rates in both study groups were not statistically different, and reached more than 85% in 10 months after the start of therapy, and in 8 months after RS healing. The WS healings in group A were recognized after 84.4±10.1 (mean±SE) days and those in B were after 100.0±12.1 days, after RS healings had been recognized, where no statistical difference was noticed between the two study groups. The results have suggested that the reduced cimetidine dosage of 400mg/day at bed time for about 6 to 8 months seems preferable to introduce the complete (WS) healing of gastric ulcer from its incomplete (RS) healing.
