<p>[Purpose] We investigated the efficacy of our original external fixation target system in ultra-widefield (UWF) fluorescein angiography (FA) for diabetic patients.</p><p>[Subjects and Methods] Peripheral area capture and obtaining time using our external fixation target system on UWF FA in 36 eyes of 18 diabetic patients were compared with those from verbal instructions instead of our fixation system in 36 eyes of 18 diabetic patients as a control group. Our external fixation target system comprised light-emitting diodes and phosphorescent tape on a board set at a viewing angle of 40° in front of the patient. The peripheral area capture and obtainig time of 4 peripheral areas were compared between our external fixation target system and the control group.</p><p>[Results] The peripheral area captured with our external fixation target system was 2,661,692 ± 453,261 pixels, significantly wider than the 2,102,607 ± 474,800 pixels in the control group (p<0.05). Obtaining time was 50 ± 8 s in our external fixation target system group, significantly shorter than the 79 ± 36 s in the control group (p<0.05).</p><p>[Conclusions] Our external fixation target system on UWF FA seems promising, allowing capture of a wider retinal field in a shorter time.</p>
