A Case of Solar Urticaria Induced by UVA.

Abstract

A 25-year-old woman with solar urticaria is described. She had been suffering for three years from erythema and wheal formation on her neck, upper chest and upper arms, when exposed to the sun light for about 10 minutes or longer. They appeared during sun exposure and then disappeared an hour later. Phototesting using glass filters suggested the presence of an action spectrum ranging from 320 to 390 nm and from 390 to 480 nm. However, phototesting using a monochromator demonstrated that light ranging from 340 to 400 nm induced erythema and that the strongest reaction was induced when the wave length was 360 to 370 nm. The plasma histamine level in the blood circulated through the UVA-exposed area was elevated and the oral administration of ebastine reduced the urticarial reaction, thus suggesting mast cell degranulation in association with UVA irradiation which thus induced the wheal-and-erythema formation. However, both a passive transfer test and reverse passive transfer test, in which her father also participated, did not suggest the involvement of specific IgE. Tolerance could be induced by the daily irradiation of UVA in combination with the oral administration of ebastine.
