Description

We report an 80-year-old Japanese male with anti-p200 pemphigoid. He presented with disseminated edematous erythema over his entire body. Small tense blisters were observed on the erythematous plaque having a partial annular pattern. Histological examination revealed subepidermal blisters and infiltration of eosinophils and lymphocytes in the blisters and the upper dermis. Direct immunofluoresence revealed linear deposits of IgG and C3 at the basement membrane zone. Indirect immunofluorescence using 1M NaCl split skin showed that IgG in the serum reacted exclusively with the dermal side of the split. The patient's serum reacted with a 200kDa protein on immunoblotting using dermal extracts. The patient responded rapidly to treatment with oral predonisolone (30 mg/day).
