A Case of Eruptive Xanthoma

Abstract

A 29-year-old male presented with white to yellowish pruritic papules on his back and extremities which increased in number over time. Most papules were localized to the hair follicles. The patient was concurrently being treated at the psychiatric unit for manic-depressive psychosis, one of the symptoms of which evidently involved the abnormally large consumption of various foods, including highly sugared soft drinks (2 to 3 litres at a time) and large quantities (upwards of ten or more servings) of cake and other sweets. The laboratory findings showed an increased level of triglycerides (5495 mg/dl), VLDL (4700 mg/dl), chylomicron (6480 mg/dl) and hyperglycemia (314 mg/dl). The patient was diagnosed with type V secondary hyperlipoproteinemia. A histopathological examination revealed a mass of foamy cells surrounding the hair follicles. Overeating or diabetes are thought to be responsible for the chronic elevation of serum lipids and the concomitant papule formation mentioned above. Control of serum glucose level through diet and anti-hyperlipoproteinemic treatment succeeded in mitigating the eruptions quickly.
