Description
A 62-year-old woman presented with 5-day history of erythematous plaques and bullae. The lesions first appeared on the right leg and then rapidly spread over the trunk and extremities. Examination revealed multiple, thumb-tip-sized, pruritic erythematous plaques in a round-like shape with small bullae on their margins. Autoimmune bullous dermatosis or drug eruption was suspected, and a biopsy was taken. Histopathological findings revealed subepidermal bulla and the microabscess consisting predominantly of neutrophils and occasional eosinophils in the dermal papillae. Direct immunofluorescence showed linear deposition of IgA and C3 at the dermo-epidermal junction. IgG was also detected as granules in some areas. Indirect immunofluorescence on 1M NaCl-split human skin showed positive reaction of IgA on the epidermal side of the skin. A diagnosis of linear IgA bullous dermatosis (lamina lucida type) was made. The patient was treated by the combination of diamino-diphenylsulfone (DDS), betamethasone and minocycline hydrochloride. Betamethasone was effective to induce remission, whereas DDS was effective to maintain remission. Effectiveness of minocycline hydrochloride was not determined.
