Two Cases of Lupus Erythematosus Tumidus Successfully Treated with Topical Tacrolimus

Lupus erythematosus tumidus (LET) is a rare variant of chronic cutaneous lupus erythematosus. Histologically, LET is characterized by abundant interstitial mucin deposition. We report two cases of LET successfully treated with topical tacrolimus. Case 1 : A 57-year-old man presented with erythematous plaques on the cheek since one month previously. Based on the histopathological findings, the diagnosis of LET was made. Although topical corticosteroid had no effects, topical tacrolimus alleviated the symptoms. Case 2: A 53-year-old man presented with erythematous plaques on the face since one year previously. He noticed exacerbation of the lesions after increased sun exposure. Based on the histopathological findings, the diagnosis of LET was made. Topical tacrolimus alleviated the symptoms. Treatment of LET is mainly based on topical corticosteroids. However, topical corticosteroids have limited effects on LET, and should be carefully monitored for side effects, such as atrophy and telangiectasia. Although few cases of LET treated with topical tacrolimus have been reported in Japan, topical tacrolimus was an effective treatment without side effects in the present cases. Topical tacrolimus could be useful for the treatment of LET.
