A Case of Leg Ulcers due to Juvenile-onset Deep Venous Thrombosis of the Leg Associated with Congenital Stenosis of the Inferior Vena Cava

Abstract

A 50-year-old man was admitted to our hospital with a 31-year history of refractory ulcers of his left lower leg. He was diagnosed as deep venous thrombosis (DVT) of the left leg when he was 14 years old. Although he was recommended to take warfarin potassium, he failed to keep taking it. Skin ulcers first developed on his left lower leg at the age of 19, and he had been treated with conservative medical treatments since then. He was referred to our department for his enlarged leg ulcers at the age of 45, and the lesions improved by bed rests and re-adminstration of warfarin potassium. At that time, DVT of his right leg also developed, and the varices of his left chest and abdomen became evident. Computed tomography scanning conducted at the age of 50 revealed marked stenosis of the inferior vena cava, which must be associated with juvenile-onset DVT of his leg leading to refractory leg ulcers. We believe that leg ulcers due to early-onset DVT may indicate the presence of congenital venous anomalies.
