A Case of Follicular Lymphoma Presenting as Cryoglobulinemic Purpura

A 45-year-old woman had a 3-month history of reticular erythema, purpura, and ulcers with crusts on the lower legs and inner sides of the feet. Histopathological findings revealed an increase of small vessels and amorphous eosinophilic material occluding expanded vessel lumens. Laboratory tests showed an elevated serum IgM (599 mg/dl) and positive results for cryoglobulins, and IgM λ-type M proteins were observed on immunoblotting analysis. Lymphoid follicle formation and large lymphocytes were observed in biopsied specimens from cervical lymph node and bone marrow. Large lymphocytes were positive for IgM and λ-chain. Taken together, monoclonal type I cryoglobulinemia caused by follicular lymphoma was diagnosed. After combination chemotherapy, she achieved complete remission with concurrent disappearance of serum cryoglobulin, accompanied by re-epithelialization of skin ulcers.
