Abstract

We investigated the efficacy of four weeks of treatment with either of the Kampo prescriptions, Japanese herbal medicines, unseiin or shimotsuto on subjective symptoms such as pruritus, severity of the skin disease, and quality of life (QOL) in atopic dermatitis patients. These patients had at least medium-grade symptoms that had not improved following previous standard treatments. There were eight patients in the unseiin group and seven in the shimotsuto group. There was significant improvement in all of the surveyed subjective symptoms, such as pruritus, a feeling of skin dryness, and sleep disorders in the unseiin group. In contrast, no significant improvement in subjective symptoms was recognized in the shimotsuto group. In particular, there was a significant difference in the change from baseline in the feeling of skin dryness between the two treatments. The SCORAD score, which is an index of the severity of skin disease symptoms, showed significant improvement only in the unseiin group. No significant change was seen in the level of the Th2 chemokine TARC before and after treatment in either group. However, one patient in the unseiin group who presented with medium-grade symptoms prior to treatment developed a slight illness after the treatment. In a QOL evaluation using the Skindex-16 questionnaire, the categories of “Total”, “Symptom”, and “Feelings” showed meaningful improvements in the unseiin group, and those of “Total”, “Feelings”, and “Function” showed meaningful improvements in the shimotsuto group. The categories that showed the most improvement in the unseiin group were “Symptom” and “Feelings.” Unseiin is produced from the common crude drugs of both orengedokuto and shimotsuto. Orengedokuto has anti-inflammatory effects, while shimotsuto has moisturizing effects. In summary, the results indicated that unseiin could reduce dermatitis more effectively than shimotsuto, and both of these treatments contributed to improving QOL in atopic dermatitis patients.
