A Case of Pyoderma Gangrenosum Associated with Myelodysplastic Syndrome

A 55-year-old man presented with multiple pustules on the extremities simultaneous with the onset of fever that soon formed painful, undermining ulcers. Despite administration of antibiotics, the fever persisted, and the ulcers became worse. The patient had been suffering from myelodysplastic syndrome and treated for one year at the hematology section of our hospital. He was referred to the dermatology section in September 2006. Pyoderma gangrenosum was first suggested from the skin symptoms. A histopathological examination of the ulcers revealed infiltration of neutrophils in the whole dermis. The diagnosis of pyoderma gangrenosum associated with myelodysplastic syndrome was established. Prednisolone improved the fever as well as the skin symptoms. The present case was a typical example of ulcerative pyoderma gangrenosum that showed pustules and rapid formation of ulcers together with the onset of fever. A review of similar casesreported in Japan from 1987 to 2011 revealed 55 such cases, and showed that the disease occurred more frequently in malesthan in females, and that main treatment included oral and/or intravenous administration of corticosteroids.
