History and perspective of cleft palate treatment.

Abstract

Looking back at the history of cleft palate surgery, there has been a shift from operating to just close the cleft palate to operating today which consider speech. And nowadays, surgery considering better speech and satisfactory maxillo facial growth has been attempted. During growth, patients with cleft palates come across many problens, for example, poor eating abilitiy, poor speech development, hearing loss, inferior maxillo growth, facial distortion, heredity disadvantages, and other complicated diseases. These problems may cause severe psychological frustration and complicated social maladaptation. Moreover, these patients need long-term care and follow-up. Naturally, a team approach which consists of many specialists has been emphasized in cleft palate treatment. However, at present in Japan, team approach treatment suffers from a number of limitations.<br>Several proposals can be suggested to overcome these limitations. First, it is desirable that centers for cleft palate treatment be established in each region in Japan. Here patients could receive consistent treatment, adequate guidance, and consultation from birth to adulthood. Second, each staff member of the cleft palate treatment team should become more specialized. Third, besides, each staff member should work to realize a treatment system centered on patient's needs oriented in daily clinical activities.
