Description

We have experienced a case of AIDS-related Kaposi sarcoma developed on the sole of the right foot. HAART treatment and Doxil® were effective. The patient was a 39-year-old male from the Republic of Guinea. There was nothing particular in his medical history or family history. One month before his first visit, a reddish swelling appeared on the sole of the right foot during his stay in Angola. He had the affected part incised (excised ?) and took medicine, but it was ineffective, and a tumor appeared in the lesion. After these episodes, he visited our hospital. On examination, a reddish tumor 32mm by 31mm in diameter, elastic rigid on palpation was seen on the sole of the right foot. Several irregular shaped nodules were seen within a purplish brown macule surrounding the tumor. Skin biopsy from the tumor showed irregular blood vessels and proliferation of endothelial cells in the stroma. The tumor cells were positive for Vimentin, CD31, CD34 and factor VIII. Also, the latent protein of HHV8 was detected. By blood examination, HIV-1 was detected by Western blot. HIV RNA was 25000copy/ml and CD4 count was as low as 41/ul. From the result, we diagnosed this case as AIDS-related Kaposi sarcoma. We started Doxil® in advance to HAART therapy, and complete remission was obtained for Kaposi sarcoma.[Skin Cancer (Japan) 2011 ; 26 : 98-102]
