Description

The patient was a male in 40s who had repeated abdominal pain and vomiting since childhood. In April 2004, a surgery for chronic idiopathic intestinal pseudo-obstruction was implemented. In October 2007, abdominal pain and vomiting occurred. Whirl sign was confirmed with an abdominal CT and diagnosed as small bowel volvulus; and an emergency surgery was performed. As a finding at laparotomy, necrotic small intestine with 180-degree torsion was observed. The small intestine was removed, and a small bowel fistula was formed at the proximal side of the previous ileostomy. Postoperative course was satisfactory, and the patient was changing hospital on 33rd days. <br>Here we report our experience of small bowel volvulus diagnosed from the whirl sign after a surgery of chronic idiopathic intestinal pseudo-obstruction with characteristic abdominal CT images as well as some bibliographic consideration.
