A Case Report of Acute Appendicitis with Appendiceal Calculus Treated by Laparoscopic Surgery

<p>Acute appendicitis with an appendiceal calculus is relatively rare, and it tends to be severe. However, the severity of appendicitis reportedly changes according to the location of such calculi. We describe acute appendicitis accompanied by a calculus at the tip of the appendix that was treated by laparoscopic surgery. A 43-year-old man presented at our hospital with right lower abdominal pain. Laboratory findings showed the absence of an inflammatory response, but enhanced abdominal computed tomography revealed wall thickness and a calculus at the tip of the appendix. We diagnosed acute appendicitis with appendiceal calculus, and performed laparoscopic appendectomy. The resected specimen featured a black calculus and a thickened wall at the tip of the appendix. Histopathological findings revealed phlegmonous appendicitis. The appendiceal calculus comprised carbonate apatite and calcium salts of fatty acids. This is the first report of an appendiceal calculus with a carbonate apatite component. The postoperative course was uneventful, and the patient was discharged from hospital four days after surgery.</p>
