Description
An 89-years-old woman noticed large mass located in a whole left breast. The mass was well morable and elastic soft. Ultrasonography showed papillary proliferation inside a huge cyst 13cm in diameter. Fine needle aspiration of the lesion revealed ductal carcinoma cells. Intracystic CEA and CA15-3 values were abnormally high, and those of serum CEA and CA15-3 were also far higher than were normal. There was no Clinical metastasis or other disease. A left mastectomy with sentinel lymph node biopsy was performed. The pathological examination revealed invasive ductal carcinoma, papillo-tubular carcinoma without lymph-node metastasis. Tumor cells were ER-positive, but PR-negative and HER2-strongly positive. Intracystic papillary carcinoma of breast (IPC) is rare and often difficult to be correctly diagnosed. However, this kind of tumor is reported to have high hormone sensitivity and good prognosis. The examination of CEA and CA15-3 value in both serum and intracystic fluid was very useful for the correct diagnosis. In addition, our case had a diagnosis of IPC with invasive, ER and HER2-positive type. Further studies are required to reveal the etiology of the disease.
