Giant bullae were resected in 86 patients who were examined by lung function tests and ventilation scintigraphy with 133Xe. Pneumothorax, respiratory failure and infection of a bulla were the main preoperative complications. In the older age group with preoperative complications, 5 patients died post-operatively. For giant bulla, surgical intervention should be performed in the early stage to prevent such complications and preserve lung function.<BR>We classified 43 cases of giant bulla into non-communicating (15) and communicating bulla cases (28). As most patients with non-communicating bullae had symptoms and decreased lung function preoperatively, statistically significant improvement of lung function was noted postoperatively. The improved postoperative lung function was maintained for a long period. Patients with severe obstruction tended to deteriorate gradually. We evaluated the effectiveness of surgical therapy for giant bullae.<BR>Patients with giant bullae located in the middle or lower lobes had severe deterioration of lung function, and these patients showed significant post-operative improvement.
