The patient was a 57-year-old man with malignant lymphoma around the upper part of sternum, who visited us with the complaints of pain and a palpable mass.<BR>Resection of the tumor involved removing all layers of the chest wall, including the upper half of the sternum, anterior segments of the first three ribs bilaterally and the proximal one-third of both clavicles, thymus, subcutaneous tissue and skin as well as a portion of the left upper lobe, pericardium and pleura invaded by the tumor. The defect, 11 by 12 cm, was repaired with double marlex mesh and a musculocutaneous flap from the left pectoralis major.<BR>As there was paradoxical motion of the reconstructed chest wall, it was necessary to intubate and apply ventilatory support for 20 days.<BR>The wound became infected, but antibiotic therapy controlled the infection.<BR>It seemed that a more rigid prosthesis should be made for reconstruction after resection of the sternum, although Marlex mesh has good compatibility with host tissues.
