The purpose of this study was: 1) to compare the amount of endotoxin obtained by subgingival irrigation in several morphopathological levels of the periodontal pockets, and 2) to evaluate the clinical effect of the subgingival irrigation.
Sixty-four periodontally involved teeth from 49 patients were clinically evaluated on probing depth, Gigival Index (GI) and Gingival Bleeding Index (GBI) at their first visits. One week later, subgingival irrigation was performed with 50 ml of distilled water after inserting a syringe needle in the periodontal pocket at 3 mm apically from the gingival margin. The water was subsequently collected. Following sonication of each sample, the amount of protein was determined. Endotoxin extraction technique with hot phenol-water method was used for their determination.
Scaling and root planing was performed on two teeth each of the 15 out of 49 patienes. Subgingival irrigation was carried out once every week during one month on one of the tooth, leaving the other tooth without subgingival irrigation. One month later, endotoxin in the subgingival irrigated water of their periodontal pockets wa determined. Clinical findings were also recorded at that time, and the following results wer obtaine:
1. Statistically significant relation was recognized between their clinical findings and the amount of endotoxin obtained with the subgingival irrigation in the periodontal pockets.
2. After scaling and root planing, endotoxin decreased with the improvement of the clinical findings.
3. Subgingival irrigation had more positive effect for the reduction of endotoxin and the improvement of clinical findings (GI and GBI), compared with other data obtained after scaling and root planing.
