Chronic lung disease (CLD) is serious ploblem in patients with bronchopulmonarydysplasia (BPD). Thirteen infants with CLD caused by BPD underwent bronchography andscopy to determine if airway abnormalities were contributing to prolonged respiratorysymptoms. Examinations revealed an airway stenosis in various degree in all patients. Therewere 2 distinct types of airway stenosis. One was “type T”: stenosis at the subglottis, thetrachea, and the mainstem bronchi that were found in 7 patients. The other was “type S”:stenosis at the lobar, segmental, and subsegmental bronchi in 6 patients. Clinically therespiratory problems were more serious in the patients with “type T” than with “type S”. Inall individuals with “type T”, surgical treatments were necessary, including a tracheal end toend anastomosis in 3 patients, a balloon dilation in 3, and a tracheostomy in 2. We concludedthat the most important pathological change in infants with CLD caused by BPD is a severestenosis of central respiratory tract, especially from the subglottis through the mainstembronchi.
