Central vein catheter associated blood stream infection (CVCA-BSI) surveillance was continued in all wards of our hospital for 18 months from June 2004 to evaluate the occurrence of CVCA-BSI in our hospital and to create more effective countermeasures. The total number of inpatients undergoing CV catheterization was 2, 290, with no seasonal changes in catheter use among the inpatients in each ward. A total of 176 CVCA-BSI cases (8%) were confirmed. The incidences of CVCA-BSI were 6.1% in the medical ward, 6.0% in the surgical A ward, 2.9% in the surgical B ward, and 1.4% in the ICU, so the incidence in the medical ward was higher than in the ICU. Assessment of the medical ward found the compliance with maximal barrier precautions during catheter insertion was over 71% and the best among the 4 wards. However, Staphylococcus epidermidis was dominant at 72.2% among the isolated bacterium species in the medical ward, suggesting that the procedures of insertion and management of the catheter, and combination of drugs should be improved. Efforts to reduce CVCA-BSI must continue based on targeted surveillance, especially in wards with many patients requiring catheterization.
