I seems to be well accepted that the behaviors of fibrin/fibrinogen<br>products (FDP) and th D-dimer do not always associate with each other insamples derived from patients with DIC. The aim of this study is to clarifythe behaviors of these two parameters in 42 patients with DIC. The level of FDP in serum and that of D-dimer in plasma were measured in these patients, respectively, at the onset (day 1) and during its propagation on day 3. Out of these 42 patients, 18 were found to have the mulptile organ dysfunction syndrome (MODS), and the average interval between the onset of DIC and that of MODS was1.1±1.7 days(mean+SD). The remaining 24 were not associated with MODS. Although there was no significant difference between the levels of FDP in these two groups, there was a significant increase in the level of D-dimer in the MODS group as compared with that in the non-MODS group(p<0. 01). Furthermore, the D-dimer was found to be increased before the onset of organ dysfunction in the MODS group, and thus the D-dimer appears to be more useful than FDP for predicting the prognosis of DIC. The D-dimer/FDP ratio may also function as a powerful tool for the prediction of MODS. In conclusion, the measurement of D-dimer and estimationof the D-dimer/FDP retio may serve as markers for the prediction of organ dysfunction in patients with DIC.
