Comparative study of bleeding time evaluation: Ivy and Simplate method in Japanese healthy subjects

The bleeding time method has been used to evaluate the antihemostatic effect and to predict bleeding complications, if any, in relation to the newly developing antiplatelet drugs. Although the Simplate bleeding time method has mostly been used for this purpose in our country, scar formation in the skin has caused a serious problem accompanying this method. As has widely been attempted in Europe and the United States, the Ivy method using a thin taper needle seems to be a less aggressive method and thus to be evaluated in our country as well. In this study, we examined the effects of antiplatelet drugs, i.e., acetylsalicylic acid, ticlopidine or a combination of both on the two bleeding time methods in 20 to 40-year-old 55 healthy males. Namely, 330 mg of acetylsalicylic acid, 200 mg of ticlopidine, or a combination of both was given daily over a period of 5 days to 19, 18 and 18 male subjects, respectively, and the bleeding time was measured by the two methods in each subject, and the results were compared. Although administration of acetylsalicylic acid alone or ticlopidine alone gave longer bleeding times only in the Simplate method, combined administration of the two drugs significantly prolonged the bleeding times measured by both methods. Furthermore, substantial prolongation of the bleeding time by the Ivy method was observed in thrombocytopenic patients. We thus propose that the Simplate method can be replaced by the Ivy method that is much less aggressive to the patients.
