It has been noted that mammography requires comparison with previous films taken at routine check-ups. However, previous mammograms are not always available when in the custody of the local government. We investigated the benefit of comparative mammography under the system for ordering previous films adopted by Ibaraki Health Service Association, only when they are required. We assessed the frequency of such film comparison, the findings obtained, and the results in 2890 consecutive cases reported between July and December 2001. The need for further examination was suggested in 115 cases (4.0%), and breast cancers were revealed in 12 cases (0.42%). Comparative mammograms were recommended in 57 cases, comprising 4.3% of a total of 1317 subjects who received an annual check-up. The number of cases for which further examination was suggested decreased after comparative mammography from 146 (5.1%) to 115 (4.0%). Two breast cancers were discovered among 26 cases for which comparative mammography was done. No breast cancers were found during two years of follow-up in the 31 cases assessed as requiring no further examination. Focal asymmetric densities were mostly demonstrated by comparison with previous radiographs in approximately two thirds of the cases. This indicates the benefit of comparative mammography for minimizing the number of cases for which further examination is suggested, as well as for increasing the accuracy of diagnosis. Our established system allowing prerequisite comparison with former films may improve the utility of its diagnostic strategy, since the issue of reference to all previous films still remains problematic.
