Description

The effectiveness of speech audiometry in the diagnosis of functional hearing loss was reviewed in young patients. We examined the relation of pure-tone hearing to the speech recognition threshold (SRT) and the maximum speech discrimination score degree in 38 ears of 20 functionally deaf children examined at Nara Medical University Hospital during a three-year period. Moreover, characteristic responses to the speech discrimination test (SDT) were examined. In the comparison between pure-tone hearing and SRT, SRT was lower than pure-tone hearing in 74% of the cases examined. In the comparison between pure-tone hearing and the highest degree of distinctness, there were many children in whom the degree of distinctness was comparatively maintained, though even pure-tone hearing was poor. Based on these findings, patients with functional hearing loss tend to show better hearing in speech audiometry than in pure tone audiometry. It is thought that there are patients whose word sound hearing tends to be better than their pure sound hearing. In addition unnatural responses in speech audiometry also help to diagnose functional hearing loss.
